Getting T-shirts printed

I have come up with a little design of my own which I would like to get printed onto some T-shirts, only a limited run of upto 5, with the design on the front (only a small design, probably about (30cm x 10cm), and get "Blink" printed on the back. Anyone know where I could get this done cheaply but good quality (ie not those iron-on transfers)
