Usually things like:

 

1) What did u think of the record (Rate 1 - 10)

2) Crowd reaction - (Rate 1-10)

3) Will you chart this release?

4) Preferred mix?

5) Other comments.

what about on the advertising sheet too, what normally gets told there for you other than:

 

Artist/Title

release date

distribution

sales notes

dj support

press

radio, tv and advertising
